This costume took very little, a white henly, some white shorts, white thigh highs, white suspenders. Black hat, boots, and cane. The key is the makeup

Personal Thoughts:

This one went over a lot of peoples head, but the Stanley Kubrick classic A Clockwork Orange speaks for itself, google it, then check out the costume.
